Rumford Hospital’s Ambulatory Diabetes Education Follow-up program (ADEF)

Ambulatory Diabetes Education Follow-up program is offered through Central Maine Medical Center with a satellite site at Rumford Hospital. Diabetes Educators Anne Marie Mahar, RN, Jane Wardwell, RD, and Robin Andrews, RDT, offer this program to clients with a physician’s referral.

Diabetes education helps people acquire new skills in nutrition, exercise and caring for themselves when they are diagnosed with diabetes, whether their diagnosis is recent or long-term.  Clients learn about blood glucose monitoring, medications and stress management.  The educators at Rumford Hospital offer updates on the newest findings in diabetes care.

Once a primary care physician has referred a client to the Ambulatory Diabetes Education and Follow-up program, the clients meets with a registered nurse to assess knowledge and lifestyle and to set goals for diabetes education.  Next the client meets with a registered dietitian to develop an individualized meal plan.  Classroom education follows through interactive television from Central Maine Medical Center with Anne Marie Mahar as the on-site educator.  At Central Maine Medical Center are Monique Hebert, RN, CDE; Tina Love, RN, CDE; and Ellen Dionne, RD, CDE, all certified diabetes educators.  The diabetes educators also follow the client at one, three, six and nine months to review and revise the client’s goals and assist with any problems or concerns.

Anyone with diabetes is eligible for the program and a family member is encouraged to attend the sessions at no additional cost.  For more information about diabetes education, call 369-1127 or your physician.
